Understanding Bacterial Vaginosis:

Understanding Bacterial Vaginosis:
Bacterial vaginosis occurs when the natural balance of bacteria in the vagina is disrupted. A healthy vagina is dominated by Lactobacillus bacteria, which maintain an acidic environment that protects against infection. When this balance shifts and other bacteria, particularly Gardnerella vaginalis, multiply and establish themselves, the result is BV.
BV is not a sexually transmitted infection, though sexual activity can disturb the vaginal environment and increase risk. It is the most common vaginal condition in women of childbearing age and is not caused by poor hygiene.
Common risk factors include:
- Using scented soaps, shower gels, or douches in the vaginal area.
- Smoking, which alters the vaginal environment.
- Using an intrauterine device such as a coil.
- Having a female sexual partner with BV.
- Washing underwear with strongly scented detergent.

BV Symptoms: WHAT TO LOOK OUT FOR
BV Symptoms: WHAT TO LOOK OUT FOR
Bacterial vaginosis does not always cause symptoms. Around half of all women with BV will have no noticeable signs at all, meaning it is regularly found only when testing is carried out for another reason.
When symptoms do occur, the most recognisable is an unusual vaginal discharge. BV typically causes a thin, watery discharge that is grey or white. It is often accompanied by a distinct fishy smell that tends to become more noticeable after sex. This distinguishes BV from thrush, which usually causes a thick, white discharge without significant odour.
Other symptoms that may be present include:
- Itching or irritation around the entrance to the vagina.
- A mild burning sensation when urinating.
- A change in vaginal discharge that is new or different from usual.
BV typically does not cause soreness, redness, or swelling. If you have those symptoms alongside discharge, an STI or thrush may also need to be considered.
Because BV symptoms overlap with other conditions, a clinical assessment and swab test at our private clinic will give you an accurate diagnosis rather than a guess, so we can start the right treatment the same day.

BV Treatment: WHAT HAPPENS AT YOUR APPOINTMENT

BV Treatment: WHAT HAPPENS AT YOUR APPOINTMENT
If your private consultation confirms BV, treatment can begin immediately. Doctors treat bacterial vaginosis with antibiotics and prescribe the most appropriate option based on your history and circumstances.
The most commonly prescribed treatment is metronidazole, available as a course of oral tablets taken over five to seven days or as a vaginal gel applied nightly for five nights. Clindamycin cream, inserted vaginally over seven nights, is an effective alternative for those who cannot take metronidazole.
Recurrent BV is common, and some women experience multiple episodes each year. If this applies to you, your doctor will discuss options for managing recurrence, which may include a longer course of suppressive antibiotic treatment and practical guidance on reducing the factors that disrupt vaginal balance.
Complete the full antibiotic course even if symptoms clear up early. Female partners should also be assessed, as they may carry and transmit the same bacterial imbalance. Male partners do not generally need treatment.
Frequently Asked Questions: BACTERIAL VAGINOSIS
BV and thrush are both common vaginal conditions, but they have different causes and require different treatments. BV produces a thin, grey or white discharge with a fishy smell and is caused by a bacterial imbalance. Thrush produces a thick, white, cottage-cheese-like discharge, often with soreness, itching and swelling, and is caused by an overgrowth of the Candida fungus. A swab test will distinguish between them. Using an antifungal thrush treatment for BV will not work.
In some cases, mild BV may resolve without treatment. However, it often returns and, if left untreated, can increase the risk of acquiring other infections including STIs. In pregnancy, untreated BV is associated with complications including preterm birth. Treatment is recommended, particularly if you have symptoms or are pregnant.
Untreated BV during pregnancy is associated with an increased risk of complications, including preterm birth and low birth weight. It may also increase susceptibility to other infections. If you are pregnant or trying to conceive and think you may have BV, it is important to seek prompt assessment and treatment.
Yes. Recurrent BV is a recognised clinical problem and affects a significant proportion of women who have had it once. Multiple episodes a year are not uncommon. Your private doctor can discuss longer-course treatment options and preventive strategies if recurrence is a concern.
No. BV is not classified as a sexually transmitted infection. It is caused by an imbalance in the vagina’s natural bacteria, not by catching an infection from a partner. However, sexual activity, including having a new partner or multiple partners, can disrupt the vaginal environment and increase the likelihood of BV developing. Female partners of women with BV may develop BV themselves and should be assessed.
If your partner is male, treatment is not required. BV is not passed to men in the same way as an STI. If your partner is female, they should be assessed as BV can be shared between female partners. Your doctor will advise you on this as part of your consultation.
Symptoms typically begin to improve within two to three days of starting antibiotic treatment. The full course should be completed even if you feel better sooner. If symptoms have not resolved after completing treatment, see your doctor for a follow-up appointment.
Yes. No GP referral or letter is required. You can book a private BV consultation directly online or by calling us. Same-day appointments are frequently available, and all care is completely confidential.
