HGV Medical Examinations with GMC-Registered Doctors
UK law requires HGV and LGV drivers to hold a valid D4 medical certificate to obtain or renew a Group 2 driving licence. Our GMC-registered GPs carry out HGV medicals at private clinics across the UK, with same-day appointments available. Each examination covers vision, blood pressure, cardiovascular and neurological function, and we issue your completed D4 form on the day. HGV medical examinations start from £75.

Who Needs an HGV Medical and WHEN

Who Needs an HGV Medical and WHEN
HGV and LGV drivers hold a Group 2 driving licence, which carries stricter medical standards than an ordinary car licence. A D4 medical examination is required in the following situations:
When applying for your first HGV or LGV licence.
When renewing your Group 2 licence. Under DVLA rules, drivers need a D4 medical at each renewal: every five years until age 45, every five years from 45 to 65, and annually from age 65 onwards.
If the DVLA requests one due to a reported health change.
If your employer or transport operator requires periodic health checks as a condition of employment.
The examination must be completed by a GMC-registered doctor. You do not need a referral from your own GP. Private clinics such as ours carry out the examination and complete the D4 form, which you then submit to the DVLA with your licence application or renewal. Most drivers book privately because NHS GPs rarely carry out D4 medicals during routine appointments.

The D4 Medical Form EXPLAINED
The D4 Medical Form EXPLAINED
The D4 is the DVLA’s official medical examination report for Group 2 driving licences. Your doctor fills it in during your appointment and signs it. Keep a copy and submit the original to the DVLA with your licence application or renewal.
The form records the following:
Vision. The DVLA requires a minimum visual acuity of 6/9 in the better eye and 6/12 in the other, with or without corrective lenses. Field of vision must also meet Group 2 standards.
Blood pressure. Readings must be below 180/100 mmHg at the time of examination. Results at or above this level may require treatment before the licence can be issued.
Cardiovascular and neurological history. Conditions such as angina, previous heart attacks, seizures, blackouts and TIAs are assessed carefully. Some conditions require a specified symptom-free period before a licence can be granted.
Other health conditions. Diabetes, sleep apnoea, renal disease and certain psychiatric conditions are all relevant to Group 2 standards and are documented accurately on the form.

What to Bring to Your HGV MEDICAL APPOINTMENT

What to Bring to Your HGV MEDICAL APPOINTMENT
To help your appointment run without delay, bring the following:
Your current driving licence, including the paper counterpart if you still have one.
A list of any medication you take regularly, including the dose and the name of the prescribing doctor.
Your glasses or contact lenses, if you use them for driving.
Details of any health conditions already declared to the DVLA.
Your D4 form if you have already downloaded it from the gov.uk website, though we can provide this for you if not.
If you have a condition that may affect your Group 2 fitness, such as controlled diabetes, a previous heart condition or a neurological history, mention this when you book. Your doctor may need slightly more time, and in some cases may ask you to bring a recent letter from your treating specialist.
If you are unsure whether a condition affects your eligibility, our doctors can advise during the appointment. We deal with these situations regularly and explain what the DVLA requires in plain terms.
HGV Medical FREQUENTLY ASKED QUESTIONS
HGV and LGV medicals are tied to your Group 2 licence renewal cycle. Drivers need a D4 medical on first application, then at each subsequent renewal. Renewals happen every five years until age 65, when your licence and the required medical become annual. Your doctor will confirm when your next examination is due.
No. You book directly with us, online or by phone. No referral is needed and no prior approval is required from your NHS GP.
The DVLA requires blood pressure to be below 180/100 mmHg at the time of examination. If your reading is above this threshold, your doctor will explain what this means and discuss the next steps, which may include medical treatment followed by a repeat assessment.
The D4 is the DVLA’s official medical examination report for Group 2 driving licences, covering HGV, LGV and bus drivers. Your doctor fills it in and signs it at the appointment. You then submit it to the DVLA with your licence application or renewal. We provide the form at your appointment, so there is nothing to source in advance.
There is no clinical difference. Both terms refer to the same D4 medical examination, required for Group 2 driving licences. HGV (Heavy Goods Vehicle) and LGV (Large Goods Vehicle) are different names for the same licence category.
If a finding from your examination means you do not currently meet DVLA Group 2 standards, your doctor will explain the situation clearly and without any rush. In many cases, treating or managing the underlying condition makes it possible to pass a repeat examination. Your doctor will advise on the appropriate next steps and, where relevant, write a referral to a specialist.
HGV medical appointments typically take 45 minutes to an hour. You will not be rushed, and there is time to ask questions at the end.
Yes. The Group 2 standard requires a minimum visual acuity of 6/9 in the better eye and 6/12 in the other. You can achieve this with corrective lenses. If you pass wearing glasses or contacts, your licence will carry a condition requiring you to drive with corrective eyewear at all times.
In many cases, yes. The DVLA has detailed guidance for Group 2 drivers managing diabetes, including requirements around hypoglycaemia awareness and blood glucose monitoring. Your doctor will assess your individual situation. Some cases require additional documentation from the specialist managing your diabetes.
